Users attempting to deploy or add questions to surveys using the UW Survey utility.

On Monday afternoon, October 11th, IT was aware of problems with the UW Survey utility.

Problem resolution:

IT personnel installed a vendor-supplied upgrade at midnight on Monday, October 19th which resolved the ongoing problems and restored normal service.
